1) Check if your smartphone or PDA needs a lanyard. Nothing's dumber than opening up your gadget for no reason at all, even if just wanted to look at its innards. Taking off the casing usually voids your warranty, and there's always some crazy soul out there who likes to open up their beloved gadget and expose the insides to willing online visitors.
2) Can you already place a lanyard on your smartphone or PDA as it is? We're being masters of the obvious here folks, but that's just to cover all bases. It goes without saying that if your gadget already has a loop designed for lanyards, you don't need to finish this post. See you tomorrow!
3) Does your smartphone or PDA use screws? In other words, is the casing fastened in place by screws? Only consider the casing, NEVER the screws that hold the electronics in place. We're looking for screws because that's where we'll be placing one end of the landyard.
4) Open up your gadget, if you haven't done so, and unscrew the chosen screw.
5) Loop one end of the lanyard around the screw.
6) While making sure the end of the lanyard stays looped around the screw, twist it back in.
7) Put the casing back on your gadget.
A few more warnings are in order: this lanyard IS NOT load-bearing. This means any violent or sudden yanking motion will probably rip off the lanyard of your smartphone or PDA. And since the lanyard is secured inside, some the casing and even the electronics may suffer some significant damage.
Last but not least, if you don't know how to handle the insides of your gadget properly, don't.

Using a screw is one way. I thought about (and rejected) supergluing a bling attachment. But my phone, which is tiny and weighs just 2 or 3 oz, has a 3.5mm audio jack socket (which I don't use much). So why not use a 3.5mm jack plug with the lanyard attched to that? Then if it is accidentally pulled too hard the plug will pull out and the phone will not be damaged. Seems to work fine for this particular phone. One minor catch is that the jack socket is not in the middle of the end of the phone - so the pretty little phone doesn't hang straight! So much simpler if the designers put an attachment hole in the right place!
